Individual Counseling, Child & Adolescent Counseling
Intensive In-Home services is a team approach designed to address the identified needs of adults and adolescents who (due to serious and chronic symptoms of emotional, behavioral, and/or substance use disorders) who are unable to remain stable in the community without intensive intervention. Providing personalized support to address individual challenges, focusing on exploring effective coping strategies and fostering personal growth. Offering age-appropriate therapy techniques tailored for children and teens to build resilience and enhance their social skills.

Diving Deeper in IIH
Intensive In-Home (IIH) service is a therapeutic approach designed to address the specific needs of adults and adolescents who, due to severe and chronic symptoms of emotional, behavioral, and/or substance use disorders, are unable to maintain stability in the community without intensive interventions.
This service is exclusively available to individuals up to the age of 20.
The medically necessary service directly addresses the recipient's mental health or substance-related diagnostic and clinical needs. These needs are evidenced by the presence of diagnosable mental, behavioral, or emotional disturbances (as defined by DSM-IV-TR and its successors), with documentation of symptoms and effects reflected in the comprehensive clinical assessment and the Person-Centered Plan (PCP).

CASE MANAGEMENT PLAN
A case management plan involves the development and periodic revision of a specific CMP, which is based on the information gathered during the assessment. It encompasses specific goals and actions aimed at addressing the medical, social, educational, and other services required by the eligible individual.

Monitoring & Follow-Up
Monitoring and follow-up encompass activities and interactions essential to ensuring the effective implementation of the CMP and addressing the needs of the eligible individual. These actions may involve communication with the individual, family members, service providers, or other entities. Such activities are conducted as often as needed to achieve their objectives.
This team service provides a variety of interventions available 24 hours a day, 7 days a week, 365 days a year. The service is delivered collaboratively by IIH staff, who maintain contact and intervene as a unified organizational team. While services are provided through a team-based approach, individual interventions may be carried out by one or more team members, as clinically indicated.
