CST Services

Community Support Teams (CST) is a Medicaid Targeted Case Management Service. These services can help you or someone eligible who have Medicaid connect with doctors, social services, educational resources, job opportunities, and other support. Regular evaluations will be done, and options will be reviewed to ensure they meet the individual's needs.

What does CST Entail?

CST is a short-term process that offers a clear and structured way to help individuals navigate change and work towards becoming self-sufficient.

The CST process involves a collaborative partnership among the consumer, the responsible party, and the case manager.

Consumers and those in charge actively participate in every stage of the process, from accessing and planning to problem-solving and finding resources.

CST ensures that available resources are efficiently evaluated and utilized in a timely and cost-effective manner.

Freedom of Choice

If you’re on Medicaid, you can choose the services that suit you best. You have the option to work with a CST Case Manager, who can help identify the support you need, assist with long-term well-being, and address daily challenges. They can also provide support for mental health or addiction issues you or your loved ones may be facing. 

Your decision will not affect your right to access other Medicaid services you qualify for. If you say yes, you pick the provider whom you will work with. You also have the right to change Community Support Team providers at any time. 

Referral & linkage

This involves activities such as scheduling appointments to help eligible individuals access necessary services. It includes efforts to link individuals with medical, social, and educational providers, as well as other programs and resources designed to address identified needs and support the achievement of goals outlined in the CMP.
